labs liability waiver

While Advanced Vitality HRT ("AV") has affiliated medical and healthcare professionals that conduct health history examinations and lab reviews, we also provide the ability for athletes and the general population to order blood work through our facility without a formal review of these labs or consultation to discuss results.

It should be understood that specific medical advice, diagnosis, and treatment will not be provided unless an appointment is requested for HRT services.

Advanced Vitality HRT does not assume liability for abnormal, critical, or potentially fatal laboratory findings if the recipient of the laboratory order does not pursue a formal consultation for bloodwork review with a licensed medical professional.

I understand that these laboratory tests are being ordered for informational purposes only and will not be reviewed by a physician or other medical staff unless a consultation is specifically requested or scheduled.

If I choose not to have a formal review, I understand and agree that:

⏺ The laboratory services are provided for informational purposes only.

⏺ AV does not provide medical diagnosis or treatment through laboratory ordering services alone.

⏺ I will seek the advice of a qualified healthcare professional regarding any medical concerns or abnormal findings.

⏺ The ordering physician and testing laboratory are independent contractors.

⏺ AV shall not be liable for acts, omissions, errors, or negligence of the testing laboratory, physician, or their employees, agents, or contractors.

If a client's result comes back critical a provider at AV is required to reach out to the patient regarding the value.
